Male Fertility and Acupuncture: Why Half the Equation Gets Ignored

Fertility is solely a female responsibility, right?

Wrong!

Male fertility is half the equation. To make a healthy baby, sperm quality is every bit as crucial as egg quality — and yet, in most conversations about fertility, the male side of things barely gets a look-in.

Here at Qi Medicine in Moonee Ponds, we see this play out all the time. Partners come in together, or sometimes the male partner comes in at the gentle insistence of the female partner, and there’s often a sense of “I’m only here because she made me.” So let’s talk about why being here — actually here, invested and proactive — genuinely matters.

Sperm health has changed dramatically — and the benchmarks with it

There are lots of theories on why this is the case — evolution, environmental factors, lifestyle — but regardless of the exact reason, if you want to have a baby, a lot more attention needs to be paid to how abundant the sperm are (count), how ready they are to swim all the way to the egg (progressive motility), and how good their form is (morphology).

We can find out the quality of sperm via a semen analysis test, which will tell us about the basic parameters — count, motility, morphology — as well as some finer details like DNA fragmentation (damage) or the presence of sperm antibodies (proteins that can interfere with sperm function).

“But I’ve had a semen analysis done and my doctor said my sperm are fine!”

Firstly — are you okay with fine? We’re here for at least good and ideally, great. “Fine” sperm can still mean there’s a slimmer chance of pregnancy happening naturally, and if it does, the probability of miscarriage may be higher.

Secondly, reference ranges for sperm parameters have shifted significantly over the last 40 years. What is considered “normal” morphology today would have been flagged as pathological in the 1990s. So “fine” really isn’t all that fine.

 

Why sperm quality matters more than you think

It can be genuinely confronting to hear that sperm health may be playing a role in your fertility journey — especially given how long we’ve collectively believed that fertility is primarily a female concern, or that a male can be fertile regardless of age or lifestyle.

While it’s true that the female reproductive system is more complex and that there’s a lot to address in getting eggs and the body pregnancy-ready, this doesn’t mean sperm health deserves any less attention.

Here are a few things worth keeping in mind:

  • It takes millions to get the job done. Yes, it only takes one sperm to fertilise the egg — but it takes millions to make that one possible. The journey is genuinely competitive.
  • Sperm DNA quality matters after fertilisation, too. From around day 3 of embryo development, after the egg has addressed what it can with its own DNA, it turns its attention to the DNA the sperm contributed. Embryos that don’t develop past day 3–5 often reflect DNA that was too damaged for the egg to work with.
  • Previous pregnancies don’t guarantee current sperm quality. Even if someone has had children already, it doesn’t mean the sperm are currently in their best shape.

 

Can acupuncture support male fertility?

Research in this area is ongoing and growing. A 2022 literature review published in Frontiers in Endocrinology — which included a meta-analysis of 12 randomised controlled trials with 1,088 participants — found that acupuncture, alone or in combination with other interventions, was associated with improvements in semen quality parameters. A 2023–2024 systematic review and meta-analysis similarly suggested that acupuncture may support sperm motility, concentration, and viability when used alongside other therapies.

Research does not yet demonstrate that acupuncture will guarantee pregnancy or resolve all causes of male infertility. The evidence base is still developing, and individual results will vary depending on the underlying cause of any sperm changes. What we can say is that acupuncture is considered safe, carries a low side-effect profile in the research literature, and may be a useful complementary support alongside your medical care.

The testes are on the outside of the body for a reason — sperm production requires a slightly lower temperature than core body temperature. Prolonged sitting, working in hot environments, wearing very tight pants, saunas, and keeping your phone in your hip pocket can all contribute to excessive heat exposure. Small changes here add up.

 

2. Keep the pipes moving. Sperm are created through a continuous production process. Ejaculating every 2–3 days encourages the testes to keep producing fresh sperm. You won’t run out — the testes produce around 120 million sperm every day on average.

3. Take alcohol and smoking seriously. Both alcohol and smoking create oxidative stress, which is associated with sperm DNA damage, reduced sperm quality, and a higher risk of miscarriage if pregnancy does occur. These are typically recommended as no-go’s during pregnancy — and the same logic applies to pre-conception, for all partners involved.
